What must I do to qualify for medical marijuana?
Qualifying for medical marijuana in Oregon takes a bit of preparation before visiting with our doctor. It's not as simple as walking into a clinic, announcing your condition, and walking out with a medical marijuana card. Oregon has a strict list of qualifying conditions and rules regarding the quality of medical documentation. Medical records transfers require adherence to HIPAA guidelines for privacy and security.

Summary of the Alternative Medical Choices medical marijuana approval process

The Oregon Medical Marijuana Program is open to patients with medical records documenting the conditions of:
- cancer,
- glaucoma,
- agitation due to Alzheimer’s disease,
- positive status for human immunodeficiency virus or acquired immune deficiency syndrome (HIV/AIDS).
Patients may also qualify with conditions that present the following symptoms:
- cachexia,
- severe pain;
- severe nausea;
- seizures, including but not limited to seizures caused by epilepsy;
- persistent muscle spasms, including but not limited to spasms caused by multiple sclerosis.

Now for those of you who would like to learn the approval process in a little more detail...
FIRST, you must determine whether you qualify. That means figuring out whether you have medical records documenting a qualifying condition that meet the following three conditions:

- Must document a qualifying condition (cancer, HIV/AIDS, glaucoma, Alzheimer's agitation, or condition causing cachexia, seizures, spasticity, severe nausea, or severe pain); and
- Must be records from treatment by a M.D. (medical doctor) or D.O. (doctor of osteopathy); and
- Must be records documenting treatment from within the past three years
SECOND, we must receive the medical records and chart notesdocumenting your conditions or symptoms covered under the Oregon Medical Marijuana Act (see Medical Marijuana Law Basics for more information) for our trained specialists to review. You have two options for getting your records to our offices:
Retrieve the records yourself (HIPAA laws make this your right - learn more here), OR
- Ask your doctor for your chart notes from your last 2-3 visits;
- Your doctor will have you fill out a form, and may charge a small fee;
- Download and fill out this form to apply to Oregon Medical Marijuana Program*;
- When you receive your records, bring them in or fax them to our office with the OMMP Application form.

- Fax both forms to Alternative Medical Choices at (503) 282–1272;
- Your doctor or clinic will fax your records to our offices.

THIRD, our trained specialists will review your medical records to verify your qualifications for medical marijuana.
- Your doctor's office / clinic can take up to 30 days by law to process your records request. Check with them first to see if they've processed the request before contacting us - any delay is usually at their end.
- When we receive your records we will contact you by phone. Within three business days we will contact you again to give you the results of your records review.
- If your records review passes state standards for medical marijuana qualification, we will then schedule you for an appointment to see our doctors. If you do not qualify, we will then help you understand why and what you can do to qualify, if possible.
FOURTH, you will schedule a brief, non-invasive exam with our doctors to verify your qualifications for medical marijuana, gauge your general health, and answer any questions you may have regarding side-effects and potential drug interactions.
- Medical personnel will record your basic vital signs;
- The doctor will conduct a non-invasive, no bodily fluids, clothed exam;
- The doctor will sign the required state form if he/she finds you qualify (if you do not qualify, you will owe nothing);
- You will pay for your clinic visit by cash, check, credit card, debit card, or money order.
FIFTH, you will take your forms to the State DHS OMMP Offices in Portland to officially apply for your medical marijuana registry card.
- Be sure you have both your Application Form and your Attending Physician's Statement.
- Your Application must list yourself (the Applicant) and a Person Responsible for a Marijuana Grow Site ("grower") and a Marijuana Grow Site Address. If you are your own grower, fill in your information in both sections A and C; however, the grow site address must be in Oregon.
- A Primary Caregiver is optional. If you have a spouse, relative, or friend who will be assisting you with medical marijuana, enter that person in section B.
- Don't forget to sign and date your Application.
- You must bring legal U.S. identification - driver's license, state ID, passport, and military ID are all valid.
- Bring a check or money order for $200 made out to OMMP. (Reduced fee of $20 if you bring proof of eligibility for SSI.)
FINALLY, once you have turned your paperwork into DHS OMMP, you are legally protected from arrest for your medical marijuana use.
- Make sure you keep the copy of your paperwork stamped with the receipt from OMMP - this will be the document that keeps you from arrest for your use of medical marijuana until your application is approved or denied by the state.
